A Lipiodol flush is a type of HSG that uses an oil-based contrast agent. In addition to evaluating the uterus and fallopian tubes, Lipiodol flushing may provide a fertility-enhancing effect in some patients.
Lipiodol is an oil-based, iodinated contrast agent used during hysterosalpingography (HSG).
A Lipiodol flush is performed as part of an HSG to evaluate the uterine cavity and determine whether the fallopian tubes are open. In selected patients, tubal flushing with Lipiodol may also increase the chance of pregnancy following the procedure.
We typically begin the HSG with a small amount of water-soluble contrast to evaluate the uterine cavity and fallopian tubes.
If the findings are appropriate, Lipiodol is then gently introduced through the cervix and allowed to pass through the fallopian tubes. The procedure is performed under fluoroscopic X-ray guidance.
The Lipiodol flush is incorporated into the HSG procedure and does not require a separate appointment.
Studies have found that Lipiodol flushing can increase pregnancy rates in some women, particularly those with unexplained infertility and those with endometriosis and otherwise normal, open fallopian tubes.
The degree of benefit varies among studies and patient populations. Lipiodol should therefore be considered an option for selected patients rather than a treatment that guarantees pregnancy.
Some studies have found that the fertility benefit may extend beyond the first several months following the procedure. The available evidence does not establish that Lipiodol improves the outcome of IVF.
Lipiodol flushing may be considered for patients undergoing HSG who:
Your physician can determine whether Lipiodol is appropriate based on your fertility history and the findings of your HSG.
Lipiodol flushing is not a treatment for a significantly blocked fallopian tube. If a fallopian tube is blocked, fallopian tube recanalization (FTR) may be an appropriate treatment in selected patients.
Lipiodol HSG is generally well tolerated, but, like any medical procedure, it has potential risks:
Contrast reaction: Allergic reactions to iodinated contrast are uncommon but possible. Please tell us if you have previously experienced an allergic reaction to iodinated contrast material.
Infection: Infection following HSG is uncommon. Your physician will determine whether antibiotics are appropriate based on your individual circumstances.
Cramping and spotting: Temporary cramping, light bleeding, or spotting may occur following the procedure.
Radiation exposure: Lipiodol HSG is performed using fluoroscopy. Radiation exposure is minimized by using the lowest dose necessary to obtain diagnostic images.
Thyroid considerations: Because Lipiodol contains iodine, patients with certain thyroid conditions may require additional consideration before undergoing the procedure.
Lipiodol may be an option if you are undergoing an HSG and would like to consider the potential fertility-enhancing effect of an oil-based tubal flush.
The decision to use Lipiodol should be individualized based on your fertility history, medical history, and HSG findings. We are happy to discuss whether Lipiodol is appropriate for you before your examination.
Lipiodol is not covered by insurance.
Lipiodol HSG: $2,300
The fee includes the Lipiodol contrast used during the procedure.
